Transfer of Medical Records

If you want to transfer your medical records to a different clinic, they will only be released after receiving a signed consent form. There will be an administration fee if it is a full set of records that you are wanting transferred. There are no charges for a Health Summary however if you are requesting access to additional information where the volume of information we hold is large, we may charge a reasonable administration fee (including fees for photocopying).
