Transfer of Medical Records

If you wish to transfer your medical records to another clinic, they will be released only after we receive a signed consent form. An administration fee will apply for transferring a full set of records. There is no charge for a Health Summary. However, if you request access to additional information and the volume of information is substantial, a reasonable administration fee (including photocopying fees) may be charged.
